The intention of this session is to present a picture of the patient's previous abilities. The therapists may make a baseline assessment of tasks, this may include; getting up and down from a chair and getting dressed, or a cognitive assessment to identity how orientated the patient is.

From the difficulties highlighted during the assessment, the therapists will negotiate meaningful goals with a patient - the main one being to regain independence.
Intervention
Intervention
Intervention focuses on programs that are both person oriented and environmental. These are designed to enable the performance of everyday tasks and adaptation of settings in which the person works, lives and socialises. Examples include; reducing environmental barriers and providing equipment which assist with independence in personal care and teaching new techniques and providingresources to lessen stress.

Therapists will evaluate the outcomes and measure them against the patient's baseline ability, in doing so they will be able to identify clearly the improvements made.

Patients are discharged either independently (with or without aid) or onto another service.
